Equipment Request Form

This form should only be used when a facility has already been reserved (i.e., equipment was excluded from original request) or for table displays (i.e., 1 or 2 tables in the Cromer Lobby), etc.

Tables needed for the female floors on Fritz-Conrad should coordinate with our office a time for equipment delivery.

 

   Allow 2-3 Days for processing. Request must be submitted at least 2 weeks in advance.

Equipment Needed

For tables indicate size: 6ft, 8ft, or Rounds

 

Flip charts and dry erase boards are purchased for use in the Cromer Center only.

Clients must provide their own markers.

 

Set-up Description:

Delivery Only - Client will set up.  You will need to provide  a picture diagram of elaborate set-ups.
