The following evaluation is very important in the continued improvement of our athletic training education program.  It is only through your honest critique that we can assist all supervisors become better educators.  This will in turn make your experience better.  you are encouraged to write specific comments at the bottom of this form.  Your name will be left anonymous.

Rating Scale:  5 = Excellent, 4 = Good, 3 = Adequate, 2 = Needs Improvement, 1 = Poor, NA = Not Applicable
